Accurate, thorough information about the patient is an absolute necessity when it comes to effective medical care. That’s why all of our new patients must fill out our comprehensive New Patient Information Packet and send the completed form to us before we can schedule your first appointment.
The New Patient Form is 18 pages long and might take around an hour or more to fill out completely. But we assure you that every bit of information we ask for is essential for us in our goal of providing you with the best care possible.
Here’s how it works:
- If you would like to establish care at Neurology Solutions as a new patient, you MUST FIRST follow the process outlined on our For New Patients page. Please contact our Scheduling Department at 512-977-7000, ext. 115, before completing the Online New Patient Information Packet. (Steps 2-6 below.)
- Click here to complete the Online New Patient Information Packet: The form will open automatically, and you can complete and submit it to us online.
- To start, click the blue button at the bottom of the screen to continue. Then scroll through the form to answer all of the questions.
- At the end of the form, click on the blue submit button, and you will be prompted to enter your email address to confirm your identity. Click the second blue submit button.
- You will receive an email from ADOBE SIGN. Please open the email and confirm your packet by clicking the link: “Confirm my email address” in the body of the email. Unfortunately, we won’t get your packet until this step is completed.
- Once we receive your packet, we will contact you to schedule your first appointment.
- WARNING: Be careful not to close the browser window before you submit the completed form as you may lose your work.
